Ciprofloxacin and dexamethasone ear drops: uses and side effects

A man with a brown shirt looking towards us smiling

Cilodex ear drops, widely available in the UK and referenced also as Cilodex in the BNF (British National Formulary), are a highly effective treatment for certain ear infections. This auricular suspension combines two active ingredients: ciprofloxacin, an antibiotic from the fluoroquinolone class that targets and eliminates bacteria responsible for infections, and dexamethasone, a corticosteroid that reduces inflammation and redness. Cilodex ear drops are used to treat acute middle ear infections (otitis media) in patients with tympanostomy tubes ("grommets") and acute outer ear infections (otitis externa, or swimmer’s ear). The formulation is particularly beneficial for alleviating pain and swelling associated with these conditions. Clinical studies have demonstrated its safety and efficacy in pediatric patients aged six months and older for middle ear infections and in children aged one year or older for external ear infections. Administering Cilodex involves instilling the prescribed number of drops into the affected ear, typically twice daily for a specific duration, as directed by a healthcare provider. The drops should be warmed in the hands before application to prevent discomfort. Completing the entire course of treatment is crucial to ensure full recovery and avoid bacterial resistance. Cilodex ear drops are a trusted solution for managing ear infections in the UK, offering targeted action against bacteria and effective inflammation relief. Always consult a healthcare professional before use. Read more in our article.

Ciprofloxacin and dexamethasone ear drops: what to know

Ciprofloxacin and dexamethasone ear drops are prescribed to treat specific ear infections. This otic suspension combines ciprofloxacin, a fluoroquinolone antibiotic, with dexamethasone, a corticosteroid, to combat bacterial infections and reduce associated inflammation.

Indications

Ciprofloxacin and dexamethasone ear drops are indicated for:

  • Acute Otitis Media (AOM): Middle ear infections in pediatric patients (aged 6 months and older) with tympanostomy tubes, caused by bacteria such as Staphylococcus aureus, Streptococcus pneumoniae, Haemophilus influenzae, Moraxella catarrhalis, and Pseudomonas aeruginosa.
  • Acute Otitis Externa (AOE): Infections of the outer ear canal, commonly known as swimmer's ear, in patients aged 6 months and older, typically due to Staphylococcus aureus and Pseudomonas aeruginosa.

Side effects

  • Common side effects may include ear discomfort, pain, or itching. Serious side effects are rare but can occur. If you experience symptoms such as rash, hives, swelling of the face or throat, or difficulty breathing, discontinue use and seek immediate medical attention.

Precautions

  • Allergies: Inform your doctor if you have a history of hypersensitivity to ciprofloxacin, other quinolones, or any components of the medication.
  • Viral or Fungal Infections: This medication is contraindicated in viral infections of the external canal, including herpes simplex infections, and fungal otic infections.
  • Pregnancy and Breastfeeding: Consult your healthcare provider if you are pregnant or breastfeeding to discuss potential risks and benefits.

Administration Tips

  • Avoid Contamination: Do not touch the dropper tip to any surface, including the ear or hands, to maintain sterility.
  • Keep Ear Dry: While using this medication, keep the infected ear(s) clean and dry. Avoid getting the infected ear(s) wet while bathing, and avoid swimming unless your doctor has told you otherwise.
In summary, ciprofloxacin and dexamethasone ear drops are effective in treating certain bacterial ear infections by combining antibiotic and anti-inflammatory properties. Adhering to the prescribed dosage and administration guidelines is crucial for optimal outcomes. Always consult your healthcare provider for personalized advice and report any adverse reactions promptly.

BNF Ciprofloxacin: dosage and administration

The standard dosage for both AOM and AOE is four drops instilled into the affected ear twice daily for seven days. Before administration, warm the bottle in your hand for one to two minutes to prevent dizziness that can result from instilling a cold suspension. Shake the bottle well immediately before use. After instillation, patients should lie with the affected ear upward for at least 60 seconds to facilitate penetration of the drops into the ear canal.

Ciprofloxacin ear drops for ear infection

Ciprofloxacin is a widely used antibiotic belonging to the fluoroquinolone family. It combats bacterial infections by eliminating the bacteria responsible for the infection. As a broad-spectrum antibiotic, ciprofloxacin treats a range of bacterial infections, including:

  • Ear infections, such as otitis externa or middle ear infections.
  • Eye infections, including conjunctivitis.
  • Uncomplicated urinary tract infections (UTIs) when other treatments are unsuitable.
  • Complicated UTIs and chest infections, including pneumonia.
  • Skin and bone infections.
  • Certain sexually transmitted infections (STIs).
  • Infections resistant to other antibiotics.
  • Prevention of meningitis in individuals exposed to the bacteria.

Ciprofloxacin is available only by prescription and comes in various forms, including tablets, liquid for oral use, and ciprofloxacin ear and eye drops. It is also administered via injection in hospital settings.

For ear infections, ciprofloxacin ear drops are commonly prescribed, often combined with other medicines like fluocinolone (Cetraxal Plus) or dexamethasone. These drops effectively target localized bacterial infections and reduce associated symptoms such as inflammation. Ciprofloxacin for ear infections is especially effective in cases where other treatments have failed or when a fast-acting solution is needed.

Similarly, ciprofloxacin eye drops are used for bacterial eye infections, relieving symptoms like redness and irritation. While ciprofloxacin tablets or liquid are potent, they are typically reserved for cases where other antibiotics are not suitable due to their potential for severe side effects.

When using ciprofloxacin ear drops or other forms of this medication, follow your doctor’s instructions carefully. Although it starts working immediately, symptoms generally improve within 2–3 days. Contact your doctor if symptoms persist or worsen.

Ciprofloxacin’s versatility makes it a valuable option for treating stubborn infections, particularly ear infections, providing rapid relief while targeting the root cause. Always discuss potential side effects and precautions with your healthcare provider before starting treatment.

How do Ciprofloxacin and fluoroquinolone antibiotics work?

Ciprofloxacin belongs to a class of medicines known as fluoroquinolone (or quinolone) antibiotics. These antibiotics work by killing bacteria responsible for infections. While many infections respond quickly, the time it takes to feel better depends on the type of infection.

When Will You Feel Better?

For most infections, you should start feeling improvement within a few days of starting ciprofloxacin. However, some infections, such as bone infections like osteomyelitis, may take longer. It might take a week or more to notice significant improvement in these cases.

If you do not start feeling better within 2 to 3 days, or if your symptoms worsen at any time, contact your doctor.

Duration of Use

Ciprofloxacin is typically prescribed for short-term use to clear infections. Due to the risk of serious side effects, it is not prescribed for more than three months. It’s essential to complete the full course as your doctor directs better before finishing.

Side Effects

While most people experience no side effects, ciprofloxacin can occasionally cause serious reactions. These side effects may occur during treatment or even months after stopping. In rare cases, they can be long-lasting or permanent.

If you have concerns about continuing ciprofloxacin, discuss them with your doctor or pharmacist.

Broad-Spectrum Antibiotic

Ciprofloxacin is a broad-spectrum antibiotic, meaning it targets a wide range of bacteria. It was previously widely used for respiratory and urinary tract infections but is now prescribed less often due to potential side effects. Your doctor will choose the most appropriate antibiotic based on the bacteria causing your infection. Narrow-spectrum antibiotics target specific bacteria, while broad-spectrum antibiotics, like ciprofloxacin, address a wider range.

Other Quinolone Antibiotics

Other quinolones include levofloxacin, moxifloxacin, and ofloxacin. These antibiotics are also used to treat chest, eye, and genital infections:

  • Levofloxacin: Taken once or twice daily, also available as an inhalable liquid for certain lung infections.
  • Moxifloxacin: Typically taken once daily.
  • Ofloxacin: Usually taken one or two times a day.

Each quinolone is tailored to specific infections, ensuring effective treatment while minimizing risks.

Always disclose your medical history and current medications to avoid potential complications. Ciprofloxacin can be highly effective, but its suitability varies based on individual health conditions. Proper medical advice ensures safe and effective use.

Pregnancy, breastfeeding and Ciprofloxacin

Ciprofloxacin and Pregnancy

Ciprofloxacin tablets and liquid are generally not recommended during pregnancy. However, ear and eye drops containing ciprofloxacin are considered safe to use.

If you are pregnant or planning to conceive, consult your doctor. They will likely suggest an alternative antibiotic that is safer during pregnancy. If you are looking for information about tinnitus and pregnancy, check our dedicated article.

Ciprofloxacin and Breastfeeding

If your baby is healthy, ciprofloxacin can be taken while breastfeeding, as advised by your doctor or health visitor. Although small amounts of ciprofloxacin may pass into breast milk, it is unlikely to cause harm to your baby. However, there is one reported case of severe diarrhoea in a breastfed baby. Other antibiotics may be preferred during breastfeeding, but the most important factor is finding a medication that works for you. Discuss options with your doctor or pharmacist. Monitor your baby for unusual symptoms, including feeding difficulties, vomiting, diarrhoea, oral thrush, or skin rash. If you notice any of these or have concerns, contact your doctor, pharmacist, health visitor, or midwife.

Ciprofloxacin and Fertility

There is no strong evidence to suggest ciprofloxacin affects fertility in men or women. If you are trying to conceive, consult your doctor or pharmacist before taking ciprofloxacin to discuss any concerns.

Important: Always inform your doctor if you are:

  • Trying to conceive
  • Pregnant
  • Breastfeeding

Herbal Remedies and Supplements

Certain supplements, such as iron (e.g., ferrous sulfate or fumarate), calcium, and zinc, can interfere with ciprofloxacin. To avoid issues, leave a gap of at least 2 hours before or after taking these supplements with your ciprofloxacin dose.

There is limited research on the safety of using herbal remedies, complementary medicines, or supplements alongside ciprofloxacin. Unlike prescription medicines, these are not typically tested for interactions with other drugs.

Important Advice on Medicine Safety

Always tell your doctor or pharmacist about any medicines you are taking, including:

  • Prescription medicines
  • Over-the-counter drugs
  • Herbal remedies
  • Vitamins or supplements

This helps ensure the safety and effectiveness of your treatment.

What is Cilodex ear drops used for?

Cilodex ear drops are prescribed for the treatment of specific ear infections:

The medication combines two active ingredients:

  • Ciprofloxacin: A fluoroquinolone antibiotic that works by killing bacteria responsible for infections.
  • Dexamethasone: A corticosteroid that helps reduce inflammation and redness.

By combining these agents, Cilodex addresses both the bacterial cause of the infection and the associated inflammatory symptoms. It's important to note that Cilodex is intended for auricular use only, meaning it should be applied directly into the ear. It is not suitable for use in the eyes, and should not be swallowed, injected, or inhaled. Always use this medication exactly as prescribed by your healthcare provider to ensure its effectiveness and to reduce the risk of side effects.

Is Cilodex ear drops discontinued?

Zoom on the ear of a red-haired man wearing a BTE hearing aid

Cilodex ear drops, a combination of ciprofloxacin and dexamethasone, have been discontinued in the UK. However, generic formulations containing these active ingredients remain available. In February 2022, the Department of Health and Social Care (DHSC) issued a medicine supply notification indicating that ciprofloxacin 0.3% and dexamethasone 0.1% ear drops were out of stock until mid-March 2022. During this period, alternative treatments such as Cetraxal Plus® (ciprofloxacin 0.3% with fluocinolone acetonide 0.025%) were recommended to manage patient needs. Despite the discontinuation of the Cilodex brand, patients requiring treatment for ear infections can still access equivalent generic medications. Healthcare providers can prescribe these alternatives to ensure continuity of care. Patients are advised to consult their healthcare professionals to discuss suitable treatment options and to ensure the appropriate use of these medications.

How to use Cilodex ear drops

Cilodex ear drops are prescribed for treating acute middle ear infections (otitis media) in patients with tympanostomy tubes and outer ear infections (acute otitis externa). They combine ciprofloxacin, an antibiotic that eliminates infection-causing bacteria, and dexamethasone, a corticosteroid that reduces inflammation and redness. Recommended Dosage: For both adults and children, the standard dosage is four drops into the affected ear canal twice daily—morning and evening—for seven days. This medication is intended solely for auricular (ear) use.

Administration Instructions

  • Hand Hygiene: Thoroughly wash your hands with soap and water before administering the drops.
  • Warm the Bottle: Hold the Cilodex bottle in your hands for several minutes to warm the suspension, which helps prevent dizziness that can occur from instilling a cold solution into the ear canal.
  • Shake Well: Shake the bottle well before use to ensure the medication is properly mixed.
  • Positioning: Lie down or tilt your head so that the affected ear faces upward.
  • Administer Drops: Gently pull the outer ear lobe upward and backward to straighten the ear canal. Carefully instill four drops of Cilodex into the ear without letting the dropper tip touch your ear, fingers, or any other surfaces to avoid contamination.
  • Post-Administration: Remain in the same position for around five minutes to allow the medication to penetrate deeply into the ear. If treating both ears, repeat the process for the other ear.

Important Considerations

  • Complete the Course: Use Cilodex for the full duration prescribed, even if symptoms improve before completing the treatment, to ensure the infection is fully eradicated.
  • Avoid Contamination: Do not touch the dropper tip to any surface, including the ear, to maintain sterility.
  • Storage: Store the ear drops at room temperature, away from moisture, heat, and light. Do not freeze. Discard any unused medicine after completing the treatment.

By adhering to these guidelines, Cilodex ear drops can effectively treat ear infections and alleviate associated symptoms.

Cilodex ear drops: dosage and administration

Old man with a hat in a blue background

Cilodex ear drops, containing 3 mg of ciprofloxacin and 1 mg of dexamethasone per milliliter, are prescribed for ear infections such as acute otitis media with tympanostomy tubes and acute otitis externa. The standard dosage for both adults and children is four drops into the affected ear twice daily—morning and evening—for a duration of seven days. To administer Cilodex properly, first warm the 5 ml bottle in your hand for a few minutes to prevent dizziness that can result from instilling a cold suspension. Shake the bottle well before use. While lying down with the affected ear facing upward, instill four drops into the ear canal. For patients with tympanostomy tubes, gently press the tragus (the small flap at the ear's entrance) five times in a pumping motion to help the drops reach the middle ear. Maintain this position for about five minutes to ensure the medication penetrates deeply. It's crucial to follow the prescribed dosage and complete the full course of treatment, even if symptoms improve before finishing the medication. If you miss a dose, do not double up; simply continue with the next scheduled dose. Avoid contaminating the dropper tip by keeping it away from your ear, fingers, or any other surfaces. If symptoms persist or worsen after a week, consult your doctor.  Always use Cilodex as directed by your healthcare provider to ensure safe and effective treatment.

Cilodex ear drops price in the UK

Cilodex ear drops, containing ciprofloxacin and dexamethasone, are prescribed in the UK for treating acute otitis media in patients with tympanostomy tubes, adults with glue ear and acute otitis externa. As a prescription-only medication, their price can vary based on the dispensing pharmacy and whether the patient is paying privately or through the National Health Service (NHS). For private prescriptions, the cost of Cilodex ear drops is approximately £12.45 for a 5 ml bottle. However, prices may differ among pharmacies. Patients obtaining Cilodex via the NHS are subject to the standard prescription charge, which is £9.65 per item as of April 2024. This charge is subject to change, and certain groups may be eligible for free prescriptions or exemptions. It's important to note that the availability of Cilodex can be affected by supply issues. For instance, in February 2022, there was a reported shortage of ciprofloxacin 0.3% and dexamethasone 0.1% ear drops, with stock expected to be replenished by mid-March 2022.  Such shortages can influence both the availability and pricing of the medication. Patients are advised to consult with their healthcare provider or pharmacist for the most current information on availability and pricing of Cilodex ear drops, as these factors can vary over time and across different locations.

Is Cilodex an antibiotic?

Yes, Cilodex is an antibiotic combination ear drop. It contains two active ingredients:

  • Ciprofloxacin - This is a fluoroquinolone antibiotic that works by killing bacteria or preventing their growth. It is effective against a range of bacterial infections.
  • Dexamethasone - This is a corticosteroid that reduces inflammation and associated symptoms like swelling, redness in the ear, and irritation.

The combination is designed to treat bacterial infections of the ear, such as acute otitis externa (infection of the outer ear canal) and acute otitis media (middle ear infection) in certain cases, particularly when there are tympanostomy tubes. Ciprofloxacin addresses the bacterial cause of the infection, while dexamethasone helps alleviate inflammation and pain. It's important to note that Cilodex is not effective against viral or fungal infections and should only be used when prescribed by a healthcare provider. Misuse of antibiotics can lead to antibiotic resistance, so it’s crucial to use them as directed.

Can I drink alcohol with Cilodex?

There are no direct interactions between Cilodex ear drops and alcohol because Cilodex is applied locally to the ear and works at the site of infection, rather than being absorbed into the bloodstream in significant amounts. Therefore, drinking alcohol while using Cilodex is generally considered safe. However, here are a few considerations:

  • Underlying Infection: If you are using Cilodex for an ear infection, it's wise to limit alcohol consumption. Alcohol can impair your immune system and potentially slow your recovery from the infection.
  • Other Medications: If you are taking additional medications to manage your condition (such as painkillers or antibiotics taken orally), check for alcohol interactions with those drugs.
  • Doctor's Advice: Always follow your healthcare provider’s guidance. If they’ve provided specific advice regarding alcohol consumption during your treatment, prioritize that over general recommendations.

If you experience any unusual symptoms or are unsure, consult your doctor or pharmacist for personalized advice.

Is Cilodex ear drops over the counter and generic?

Cilodex ear drops are not available over the counter; they are a prescription-only medication in the UK and many other countries. This means you will need a valid prescription from a healthcare provider to obtain them.

Is Cilodex Generic?

Cilodex is a brand-name medication that contains the active ingredients ciprofloxacin (an antibiotic) and dexamethasone (a corticosteroid). Generic formulations containing the same combination of ciprofloxacin and dexamethasone are available in some regions, depending on the country’s pharmaceutical regulations. However, availability of a generic equivalent can vary, and the brand-name product might be more commonly dispensed in certain areas.

If you're looking for a generic version or a similar alternative, consult your healthcare provider or pharmacist for guidance. They can help you find an equivalent product if it's available and suitable for your condition.

Ciprodex ear drops combine ciprofloxacin, a fluoroquinolone antibiotic, with dexamethasone, a corticosteroid, to treat specific ear infections. This formulation is prescribed for acute otitis media in patients with tympanostomy tubes and acute otitis externa, commonly known as swimmer's ear.

  • Dosage and Administration: For both conditions, the standard dosage is four drops into the affected ear twice daily for seven days. Before administration, warm the bottle in your hands for one to two minutes to prevent dizziness that may result from instilling a cold suspension into the ear canal. Shake the bottle well immediately before use. While lying down with the affected ear facing upward, instill the drops. For middle ear infections with tympanostomy tubes, gently press the tragus five times in a pumping motion to facilitate penetration into the middle ear. Maintain this position for 60 seconds to ensure proper distribution of the medication.
  • Precautions: Ciprodex is intended for otic use only and should not be used in the eyes, injected, or inhaled. Discontinue use immediately and consult your healthcare provider if you experience signs of an allergic reaction, such as skin rash, hives, or swelling. Complete the full course of treatment, even if symptoms improve before finishing the medication, to ensure the infection is fully eradicated.
  • Side Effects: Common side effects may include ear discomfort, itching, or a headache. Serious side effects are rare but can occur. If you experience severe reactions or if your condition does not improve after one week of treatment, seek medical advice promptly. Always use Ciprodex as directed by your healthcare provider, and consult them if you have any concerns or questions regarding its use.

Ciprodex ear drops dosage and uses

Administer this medication in the affected ear as prescribed by your doctor, typically twice daily (every 12 hours). It is for ear use only; do not use it in the eyes. Ensure the ear canal is clean and dry before application. To ensure proper dosage and minimize the risk of contamination, have someone else assist in applying the drops if possible. To reduce the risk of dizziness, warm the medication by holding the container in your hand for a few minutes before use.

Steps for Application:

  1. Prepare:
  2. Wash your hands thoroughly.
  3. Avoid touching the dropper tip or allowing it to come into contact with your ear or any other surface.
  4. Shake the bottle well before use.

For Middle Ear Infections (Children with Ear Tubes):

  1. Have the child lie with the affected ear facing upward.
  2. Hold the dropper directly above the ear and administer the prescribed number of drops into the ear canal.
  3. Press gently on the cartilage covering the ear opening 5 times in a pumping motion to help the medicine pass through the ear tube into the middle ear.
  4. Keep the ear facing upward for at least 1 minute or insert a soft cotton plug if directed.

For Outer Ear Infections:

  1. Lie with the affected ear facing upward.
  2. Administer the prescribed number of drops directly into the ear canal.
  3. For adults, gently pull the earlobe up and back. For children, pull the earlobe down and back.
  4. Keep the ear facing upward for at least 1 minute or insert a cotton plug if instructed.
  5. Repeat for the other ear if prescribed. Do not rinse the dropper. Replace the cap tightly after each use.

Usage Tips:

  • Use the medication at the same times each day to maintain consistent levels.
  • Complete the full prescribed course, even if symptoms improve, to prevent recurrence of the infection.

Ciprodex ear drops side effects

You may experience discomfort, pain, or itching in the ear. If these symptoms persist or worsen, inform your doctor or pharmacist promptly. FAQs about Cilodex and Ciprofloxacin ear drops

What happens if you leave ear drops in too long?

If you leave ear drops in for too long, especially when the ear is already inflamed, it can potentially lead to temporary issues such as swelling and discomfort. This prolonged exposure may not effectively treat the condition and could exacerbate symptoms like temporary hearing loss. It's important to follow specific guidelines for each type of ear drop to avoid adverse effects. If you experience persistent discomfort or worsening symptoms after using ear drops, consult a healthcare professional.

What is the best antibiotic for an ear infection?

Here's a list of the most common antibiotics for ear infection:

  • Amoxicillin is commonly recommended as the first-line antibiotic for ear infections.
  • Alternatives include azithromycin (Zithromax) and ciprofloxacin (Cipro), depending on the type and severity of the infection
  • Treatment should be tailored by a healthcare provider based on individual factors like allergies and previous antibiotic use

These antibiotics effectively target bacterial causes of ear infections, but specific recommendations should be discussed with a healthcare professional.

What do CILODEX ear drops do?

CILODEX ear drops contain ciprofloxacin and dexamethasone. They are used to treat infections of the outer ear canal (acute otitis externa) and middle ear infections with drainage through a tube in the eardrum (chronic suppurative otitis media) in patients who have a tube in the eardrum. Ciprofloxacin is an antibiotic that treats bacterial infections, while dexamethasone is a corticosteroid that reduces inflammation. These ear drops are applied directly into the ear canal.

How long does it take for Ciprodex drops to work?

Ciprodex drops typically begin to show improvement within one week of starting treatment for ear infections. If symptoms persist beyond this period or worsen at any time during treatment, it's advisable to consult a healthcare provider. These drops work by inhibiting bacterial growth and reducing inflammation in the ear.

How many days should you take CILODEX?

Typically, CILODEX is used for about 7 days. It's important to complete the full course of treatment even if symptoms improve before then. The duration can vary based on the severity of the ear infection and your medical condition. Your doctor may adjust the duration based on how your infection responds to treatment. Always consult your doctor if you have any questions or concerns.

Will Ciprofloxacin affect my contraception?

Ciprofloxacin does not interfere with any form of contraception, including the combined pill and emergency contraception. However, if ciprofloxacin causes you to vomit or experience severe diarrhea for more than 24 hours, the effectiveness of contraceptive pills may be reduced, and they may not fully protect you against pregnancy.

Can I drive or ride a bike with Ciprofloxacin?

Ciprofloxacin may cause tiredness or reduce alertness. It’s important to understand how the medication affects you before driving or cycling. Ear infections treated with ciprofloxacin can sometimes affect your balance. If this happens, avoid driving or cycling until you feel steady and normal again. It is illegal to drive if your ability to do so safely is impaired. You must assess whether it is safe for you to drive while taking ciprofloxacin. If in doubt, do not drive. If you’re unsure about your ability to drive safely while on ciprofloxacin, consult your doctor or pharmacist. For detailed legal guidance, visit GOV.UK for information about driving laws related to medications.

Can I drink alcohol while taking Ciprofloxacin?

Yes, you can drink alcohol with Ciprofloxacin.

What food do I need to avoid with Ciprofloxacin?

Avoid consuming dairy products (like milk, cheese, and yogurt) or calcium-fortified drinks (such as certain dairy-free milks) when taking ciprofloxacin tablets or liquid. These products can prevent the medication from being absorbed properly in your stomach. To ensure ciprofloxacin works effectively, leave at least a 2-hour gap before or after consuming these foods or drinks. After this interval, you can include dairy or calcium as part of a balanced meal. If you are using ciprofloxacin eye or ear drops, there is no need to avoid any specific foods or drinks.
